By submitting this application, I affirm that the facts set forth in it are true and complete. I understand that if I am accepted as a Head coach or a member of a coaching staff, any false statements, omissions, or other misrepresentations made by me on this application may result in my immediate termination. I understand that the position of Head Coach is an appointed position upon the approval of the Executive and any person approved and designated as such will hold that position for no definite period of time and may be terminated at any time without prior notice for whatever reasons the Executive of TBMFA deems necessary.
