ALL FAMILIES ARE TO SUBMIT THIS FORM - BLENDED AND REMOTE.  THIS FORM SHARES YOUR INTENTIONS FOR STATE TESTING THIS YEAR FOR YOUR CHILD.

If you have multiple children at IS 5 please fill out a separate form per child.  Information regarding the final dates & schedule will be shared via Pupilpath, School Messenger, Google Classroom and our school website - is5q.org.   We will continue to update families as we gather more information. Please complete this form by COB April 7th 2021.  

NYS Education Department and the Department of Education has shared that students will be taking the exams IN PERSON.  Students receiving FULLY-REMOTE instruction are not expected to come into school buildings to participate in state test administrations.  

NYS ELA Exam Dates: During the window of April 19-29, 2021
-The exam is composed of 4 reading passages and 28 multiple choice questions.  It should take an average of 85 minutes to complete.

NYS Math Exam Dates: During the window of May 3-14, 2021
-The exam is composed of 24-26 multiple choice questions. It should take an average of 85 minutes to complete.  

NYS SCIENCE EXAM Dates: During the window of June 7-9, 2021
-This exam is composed of just the single session written test only.
-There is no performance exam this year.

Once we have a final schedule of when each student will be programmed for the exam, we will be sending out an individualized email to your child(s) nycstudents.net account with the specific date of exam, time and other important testing information.

Students will not be penalized based on the family's decision of testing this year.  Please see below to complete your option of testing this year.
