Vulnerable Sector Request
We appreciate you! The Vulnerable Sector Check (Police Reference Check) provides the screening of individuals who intend on working or volunteering with vulnerable people. Please complete this form as a requirement for members over 18 years to participate in volunteering at the Apple Creek Seventh-day Adventist Church.  This is a required step to determine the next step, instruction and delivery for the Vulnerable Sector Check.  Please make an effort to provide this sensitive and valid information at your earliest convenience

DATE REQUIRE: You will be contacted for the the latest clearance letter received OR you are welcomed to provide the date under "Other".  Please select "Yes" for members who already has a valid vulnerable sector due to existing employment.  Upon completion of this form, the official document will be secured at the Secretarial office of the Ontario Seventh-day Adventist.
