Roosevelt DI 2022-2023 Registration Form
If you have more than one child interested in Roosevelt DI, please fill out the form separately for each child.

Fees:
Competitive (3rd-5th grades): $100 
Rising Stars (SK-2nd grades): $90

This fee covers team registration, program expenses, and a tournament T-shirt for every student. ​Please make the check out to Roosevelt DI (note: NOT Roosevelt Elementary) and return to the front office by Friday, Oct. 7, or you can pay via Venmo to Roosevelt DI co-coordinator Katie Ost (@Katie-Ost-2).​ There is some financial assistance available for families who need it. If the cost would prevent your child from participating, please contact Mr. Supa at ​supama@wauwatosa.k12.wi.us​.

Your child’s team may have additional small expenditures or may need you to donate supplies or snacks. We also encourage families (when possible) to support fundraisers for Friends of Tosa DI, which supports teams citywide through instant challenge kits, workshops, scholarships and funds for teams who advance to Globals. Thank you!

 If your child is in grades 3-5, please note which challenges they are interested in (you can select more than one). Students in grades K-2 will be automatically assigned to the Rising Stars challenge: Play On. All challenges are performance based and include an element of theater. We will do our best to accommodate schedule requests, but meeting times are set by the team managers. Most teams meet on weeknights, with some weekend meetings as it gets closer to tournament. Between November through March, are there days of the week when your child is NOT available to meet regularly? Note below:
DI depends on volunteers! If we do not have enough volunteer team managers, we might have to turn some students away. Team manager training is provided (tentatively scheduled for Saturday, Nov. 5, from 8:30-noon in Tosa East's library). If needed, are you willing to be a team manager or co-manager for your child's team? *
Your child must be available to compete/present at the regional tournament at Muskego High School on ​Saturday,​ ​March 11, 2022​. You should plan to attend ​all day​ if required by the team’s scheduled time slots for the instant challenge and skit (we won't know exact performance times until about a week before the tournament). For grades 3-5: If the team does well enough and advances to the state tournament, your child will compete on Saturday, April 1 at UW-Green Bay. ​The state tournament requires an overnight stay in Green Bay, and your child must be accompanied by a parent or other trusted adult. Is your child available on these dates? *
DI is a collaborative extracurricular activity that requires students to be respectful toward their teammates and managers. Listening and hard work are very important to ensure a good experience for the whole team. If managers feel a student is being disruptive, not working well with others, or not committing to practices, they may request that the child’s parents/guardians attend meetings. If behavior does not improve, they may remove the student from the team. Please review these expectations with your child and respond below after you have done so.

Photographs of DI participants may be used for publicity purposes during the year on school, PTA and Friends of Tosa DI Facebook pages. ​(Note: This is not required, but please let us know if you do NOT give permission for photos to be used in this way.) *
