Zakat Intake Form

Welcome to the Kanata Muslim Association (KMA), Zakat Assistance Online Application Form.

Should you need any assistance with filling out the form, please email Assistance@KanataMuslims.ca

The KMA Zakat committee will review your information & documents, assess your case, and decide accordingly on the type of assistance you may be eligible for.

Please note, it may between 2-4 weeks from the time of submission of this application to receive a response.

Requirements:

KMA Zakat Committee cannot assess your request without the following documents. These documents must be submitted upon request, after completing the online form.

Bank statement for the last 3 months
Credit Card statement for the last 3 months
Lease agreement
Photo ID

KMA Zakat Committee can only give assistance to cases in Ottawa or Gatineau
KMA, including its Zakat committee and all volunteers, are free from any legal liability under the Ontario Good Samaritan Act. We are volunteers that work to the best of our abilities and to the best of our intentions.

I understand that the financial assistance I may receive is limited in time duration and amount.

I understand that KMA Zakat Committee can request updated documents every 6-9 months.

I understand that if my situation changes I must inform the KMA Zakat Committee as soon as possible (within 30 days of the change).
