Provide a short and plain statement of the facts giving rise to the grievance, including the action at issue, the Articles of Incorporation, Bylaws or official written policies or procedures adopted by the Board which are alleged to have been violated by the action, the parties involved in the action, the harm to the complainant as a result of such action, and the relief sought.

Filing fee of $100 should be sent by mail to U.S. Ski & Snowboard, P.O. Box 100, Park City, UT 84060.  A complainant may request a waiver of the filing fee by emailing generalcounsel@usskiandsnowboard.org.
