Health Awareness Attestation
As an act of love and compassion toward myself and my community, I want to always remain aware and intentional about how my actions impact the health and well-being of my community, my family, and myself.  To assure that I am always as safe as possible for any interaction I may encounter as I enter the church building or whenever participating in outdoor church sponsored functions, I attest that:

Policy
This attestation needs to be completed each day that a person enters the church building or whenever participating in outdoor church sponsored functions until the Covid-19 pandemic is declared controlled.  It shall be filled out when entering the building by hard copy to be left in the folder at the east door, or on-line.  The purpose of this policy is to assure that each person has done an intentional self-assessment of his/her health at the time of entering the building or attending church sponsored functions.
