Board of Health Interest Form
Thank you for expressing interest in the Village of Greendale Health Department's Board of Health.  Our Board is made up of no more than nine members including up to seven community board members. All members need not be a healthcare provider to be appointed to the Board.  Appointments are for a three year term with the ability to be appointed for one additional term.  Being a member of the board is voluntary and much appreciated.
