This anonymous questionnaire is designed to take 3-5 minutes and provide feedback on your experience of accessing services with our surgery.
The questionnaire is divided into sections dealing with: the methods that you have used to contact the surgery, the outcome of your contact, your satisfaction with the service and your demographic details (optional). The demographic details will help us to understand how different parts of our population are accessing care.
