Please provide more information about your district to be included on cnia.org. To have your district listed, we require the following information:
  • District number
  • District location (such as state county or boundary)
  • Business meeting time and address
The other fields are optional and not required. Please do not provide full last names (last initial is encouraged) in the DCM/alt-DCM information. This includes email addresses with last names.

The form requires an email address (does not have to be Gmail) so that we may send you a copy of your response, if requested.  If we need more information, the webmaster may contact you at this email address.
