Age UK Blackburn with Darwen is a local independent charity that provides a range of services and support to older people including those living with dementia and their carers. We are keen to understand what gaps people feel that there are in local support and services following an initial diagnosis of the condition. This research will inform and shape our future service delivery in Blackburn with Darwen and will also help us influence other service provision, so we would be very grateful for honest responses to the following questions from both those living with dementia and their carers.  Thank you for your time which should take no longer than 10 minutes.
