United 304 Parent Survey

Now that the State of Illinois has moved to Phase 4 and the Illinois State Board of Education has released it's guidance for the re-opening of schools, we at United are seeking your input based on that guidance. Included in the guidance are the requirements that all people (teachers and students) must wear masks in the buildings, temperature check and symptom screenings are performed daily, and preferred social distancing of 6ft adhered to as much as possible. This data will be used to help finalize our plans. Please note that as state and ISBE guidance change, so too may our plans for re-opening. No plans have been finalized at United as of right now.
