*High School/Confirmation: Complete this form for Confirmation preparation. The Diocese of Evansville states that candidates are to faithfully attend their religious education classes in addition to Confirmation formation. Religion classes are offered through Catholic High School, the Identity Project program, and our Parish High School program held on Sunday evenings. There are separate registration forms that need to be completed for each of those programs.
