**There will be limited penning, so you must be prepared to show off your trailers. There will be no bleacher seating available. If we have to cancel the show for some reason (COVID or weather), we will contact you using the information on this form**

I consent to have my child recorded on video while in the ring. This is required by the Governor's Order to be able to have the livestock shows. If you do not, you should not attend the show. *

WAIVER OF LIABILITY:I am the parent/guardian of the minor child named below (“the participant”) and am fully competent to sign this agreement. I understand that the Participant's request to participate in the Wayne County Livestock Development Association (WCLDA) Livestock Shows has been approved. I understand that the activity involves various physical activities, including, but not limited to, contact with animals, and that there is a risk of injury associated with such activities. I understand the nature of the activity and my child's participation therein: I have explained these things to the Participant and his/her participation is both willing and voluntary. The Participant is in good health, and deemed able to participate in the activity. In addition, the Participant has adequate health insurance necessary to provide for and pay any medical costs that may arise as a result of injury to the Participant. In consideration of the Participant's being permitted to take part in the WCLDA Livestock Shows, the Participant and I assume all risks of such participation, such risks including but no t limited to, (a) accidents and/or collisions with fixed or moving objects, animals, or persons; (b) hazards caused by equipment failure; and © hazards caused by inadequate safety equipment. I further agree that neither NCSU nor its employees, officers, trustees, or agents shall be liable for any claims, demands, actions, or causes of action arising out of or in any way connected with the Participant's participation in the activity, specifically including, but not limited to liabilities, claims, demands, actions, or causes of action relating to bodily injury (including death) and property damage suffered by the Participant. Therefore, on behalf of the Participant, the parents/guardians of the Participant, and the heirs and assigns of all the foregoing, I do hereby forever release and discharge NCSU, as well as its trustees, officers, employees, and agents from all such liabilities, claims, demands, actions, or causes of action. With the activities having been explained to me, and all of my questions answered to my complete satisfaction, I consent to the Participant's participation in the WCLDA Livestock Shows and accept the facilities, premises, supervision, and equipment use in connection therewith; fully aware of the activities and risks that may be involved. I execute this consideration fully intending for myself, for the participant, and for the Participant's family, estate, heirs, administrators, personal representatives, or assigns to be bound by the same. *

We agree (and for our heirs, assigns, agents, or representatives) to be bound to forever hold harmless and release Wayne County Livestock Development Association (WCLDA), its staff, volunteers, and Board of Directors, from any and all liability or responsibility for the contraction and/or transmission of any illness, disease, or sickness related to participating in activities hosted by WCLDA. We agree to do our part by following all reasonable guidelines for personal behavior while at events hosted by the WCLDA. We agree that if we begin to experience symptoms consistent with COVID-19, or have been physically exposed to someone who has received a positive test for or diagnosis of COVID-19, we will immediately inform a staff member of the livestock shows and will voluntarily withdraw from all activities and contact with others at the event. *
