If this issue pertains to club dues, playing time, or other club-specific complaints, please contact your club director.  A listing of all club contacts can be found on the CEVA website.  CEVA has no control over these types of club-specific issues.  If you are unable to contact your club director, or your complaint involves the club director specifically, consider reaching out to another club administrator or the club's Board of Directors (if applicable).  If you are unable to contact your club director or another contact within the club, please proceed with this form by reading below.

Any sanctions levied against a member(s) will remain confidential and will not be shared with a reporting party unless legal obligations require.  Any actions taken against members are subject to CEVA's due process.

We strongly encourage this form to be completed by first-hand witnesses or participants in the incident in question.  Please include the name of any other first-hand witnesses or participants you are aware of.  Please be as specific as possible in your report.

Submissions of this form are only viewed by the CEVA staff and are treated as confidential and anonymous to the extent permitted by law.  Please note the submission of an anonymous report may impede our ability to investigate the incident in question or conduct any necessary follow-up with the reporting party.
