If you are just coming to the Jelly Bean dance and don't want to BE A COMPETITOR - just come to the dance with your $3 entrance fee and lots of Concession $$ ;-) and a very loud voice - cause the winner is determined by our noise meter. So DO NOT fill this form in if you are not one of the competitors.

This form is ONLY if you are entering their group into the competition - so that we can have your 1960's song you are performing to ready. It has a to be a 60's group (thats all - and if its full of foul language/ideals - I will call and ask you pick a more tame song please)
