Montana Youth Challenge Academy Application
The Montana Youth Challenge Academy is available to Montana youth who are at-risk of not graduating from high school or who have already dropped out. Few issues will disqualify an applicant; in most cases, accommodations are possible to help you succeed. There are NO trick questions so please answer honestly and completely.

We require a copy of the following documents to process your application.
1) Birth Certificate (or equivalent to verify your legal status)
2) Immunization Records
3) Social Security Card
4) Proof of Insurance
