Proxy Form for County Convention
This is a form to appoint a proxy to attend the Tulsa County Democratic Party County Convention on Saturday, April 3, 2021.

A delegate to the convention who is unable to attend may complete this online “Proxy Form” to a registered Democrat of the same gender from the same precinct where they are registered to vote.

This form must be completed in it's entirety prior to the start of the convention to allow the Credentials Committee to provide an Access Link to the delegates proxy. The delegates personal zoom link will not work for their proxy holder.
