By signing this form, I agree to not hold the volunteers organizing this league responsible for any injury or damages caused by my participation in activities associated with this activity.

I understand that the Women's League must follow all local and provincial COVID-19 guidelines. If there are changes in mandates around vaccination, screening and /or masking, I understand I will be required to follow these guidelines or I will be unable to participate in the league.
