We understand that life is busy and full of challenges that may prevent you from coming in person to submit excusal forms for our students. This form will allow you to report 3 absences at once Please be sure to:• Input your email• Type in your students first and last name• The reason for the absence • Date of absence(maximum 3 days)SCHOOL MAY REQUIRE DOCTOR'S NOTE AND WILL REQUIRE DOCTOR'S VERIFICATION AFTER 14 DAYS OF ABSENCE PURSUANT TO SCHOOL BOARD POLICY AR 5113.Parent must verify any Absence within (3) days. After (3) days the Absence will be permanently unexcused and considered Truancy.
