Parent Note for School Absence 2024-2025
If your child has to be absent from school a 'written note' must be provided to the school.  This form serves as the written note.

Please understand that not all parent notes are considered excused absences. There is a limit of 5 parent notes for a student.  After that, absences will require  doctor's note to be considered excused.  For more information please see the attendance page in the North Pointe handbook. 

Original medical notes must be brought to the front office or sent in with student within 10 days of the absence.
