Please note that most of the questions will not be used to determine whether or not you are accepted, but rather to ensure coverage throughout the weekend and that there are no other issues that can be worked out in advance (space needs, etc).
**COVID-19 POLICY**
All individuals coming as medics are required to present their Covid-19 vaccine card (digital copy or photo of card is sufficient) that displays vaccination to CDC guidelines OR a new unopened home Covid-19 test to perform upon check-in (you must provide the test if you choose this option, we cannot guarantee we will have tests available). If a home Covid-19 test comes back positive, that individual will not be allowed entry.
We strongly encourage everyone to take a home Covid-19 test before traveling, and we will be encouraging fans and other visitors to wear masks for this event.
