ONLY COMPLETE THIS SURVEY IF YOU HAVE MULTIPLE STUDENTS, WHO HAVE DIFFERENT LAST NAMES, AND ARE SCHEDULED TO ATTEND IN-PERSON CLASSES ON DIFFERENT DAYS.

Due to the rise in COVID19 cases across the county, state, and nation, Madison County Health Director, Chris Cook, ordered all schools in Madison County to implement a hybrid learning model beginning after the Thanksgiving holiday.

Students with last names A-K will attend on Mondays and Thursdays (Red Days).
Students with last names L-Z will attend on Tuesdays and Fridays (White Days).
Wednesday will be a virtual instruction day for all students.

We realize households may have students with different last names, which may place students under the same roof attending on different days.

PLEASE SUBMIT ONE RESPONSE PER STUDENT IN YOUR HOUSEHOLD. ALL FAMILY REQUESTS MUST BE MADE BY FRIDAY, WEDNESDAY, NOVEMBER 18TH, 2020.
