To borrow any of our books listed below, please complete this form. If you are requesting to borrow more than one title, please complete this form for each book title. 

You are responsible for ensuring appropriate use and returning all books back to the NWPTC. All books must be picked up from and returned to the address below. Once your request is received, we will contact you to determine a pick up date. If you are teaching a course that requires participants to utilize these books, please keep a record of who borrowed the book.

It is expected that all items loaned are returned in the condition it was received in. If a book has been lost or damaged beyond acceptable use, it is the responsibility of the loaner to notify the NWPTC and replace the book.
