This form is for students that fit into one of the following categories for the Summer of 2022:

1. A senior during the 21/22 school year completing graduation requirements at Rebound during the summer of 2022.

2. A high school student that is transferring to Rebound. The student does not plan to return to their high school.

3. A student wanting to take the GED test.

Current high school students need to speak to a school counselor or administration to get permission to attend Rebound.

If you plan to Return to your high school in August, DO NOT COMPLETE THIS FORM. Instead, speak with your school counselor or administrator. Information for credit recovery was sent to school counselors the first week of May.
