This form is for incidents involving allegations of a violation of MnSearch’s Event Code of Conduct. Please fill out as much information as possible based on your knowledge of the situation.

If you choose to remain anonymous, we’ll be unable to confirm with you that your report has been received or provide you with any updates as to our investigation and potential actions taken. 

Filing this report with MnSearch doesn’t constitute an official venue or police report. You’ll need to either work with MnSearch board members, event staff and/or local law enforcement to file additional reports, if you choose.

Definitions of Parties Involved

• Reporter: The person submitting the report (the complainant or a witness)
• Complainant: The person who experienced the alleged violation or harm
• Respondent: The person accused of violating the Code of Conduct
• Witness: A person who saw or heard first-hand what happened or has relevant information about the incident
