Parents are required to supply emergency/biographical contact information when their child enters MS 419. In addition, we ask families to complete Google from here so that we have a digital database which provides the school with the address and telephone number of the parents/guardians at home and work as well as a neighbor or relative so that they can be notified in the event of an emergency. If any of this information changes, please notify the school immediately so the information can be changed in the New York City Department of Education database.
