This vitally important form will take about 30 minutes to complete, you should have the names and contact details for at least 3 adults (yourself included) who can be contacted in an emergency. You will also need medical information about your child to hand.

This form should be completed FOR EACH AND EVERY CHILD as they are kept on an individual level and matched to other information from other services. We apologise for any inconvenience caused.

99% of applications can be completed online and your child will be automatically enrolled on the agreed date after you submit the form...in a very small minority of cases (for example if you do NOT give consent to some questions or do NOT agree to a statement) you may need to contact the office, the form will need to be processed differently and in person. This is because your response is considered 'non-routine' and will need to be administered separately to ensure we do not assume consent or permission for something you do not wish to give.

EXAMPLE - Statements such as" Do you agree to us providing your child with free fruit?" only have the ability for a 'Yes' answer, you can still opt-out of us providing this to your child, but would need to speak to you to understand more, hence why a 'No' answer is not available and the form will not let you proceed.
