Please complete this application if you are interested in enrolling your child(ren) in St. Mary School in Paris, Kentucky. Students will not be fully enrolled and their spot will not be guaranteed until the $150 enrollment fee is received. Please note that health records including school physical, immunization certificate, eye screening, dental screening, and birth certificate are required for all Montessori Preschool, Montessori Kindergarten, and new students.
