Region 6 - Basketball Sign-up Form

Thank you for taking the time to sign-up for BASKETBALL.

All new and returning athletes and volunteers MUST complete this form if they want to participate in BASKETBALL.
Completing this form will help us prepare for the season and provide contact information to the Head Coaches.
If your local program does not offer Basketball you may sign-up and participate with a nearby county. Keep in mind you will have to provide your own transportation.

Signing up to participate in this sport does not guarantee participation, as participation is dependent upon an adequate number of volunteers to ensure we provide safe and meaningful sports training.
