Monthly Membership Fee Agreement

What is this fee?

At Lighthouse Complex Care, we use a comprehensive, personalized medicine approach for every patient. To be able to oﬀer you this level of care, every patient at Lighthouse Complex Care is responsible for a monthly membership fee.

When does the membership fee start?

Complex Care fee starts after your initial appointment. For example, if you had an appointment before the 15th of the month you will be charged for that month. If your ﬁrst appointment is after the 15th of the month, you will be charged the following month.

When is this billed?

Complex Care fees are automatically billed at the end of each month to your saved card on ﬁle.

What does this cover?

Your monthly complex care fee represents any time that month when “out of visit” work on your case happens. It includes services like but not limited to:

●     Access to the after hours triage line;
●     Case coordination and preparation for every appointment;
●     Provider consult(Peer to Peer) with other doctors and healthcare providers;
●     Prescription management and reﬁlls in between appointments;
●     Quick calls or inquiries through the portal or info email line;
●     Any necessary case research to reﬁne care;
●     The usual referral process to other clinics and specialists;
●     Nutritional or supplement help;
●     Management of medication reactions or support when you or your loved one is sick;
●     Letters for school, work, or other social support agencies

What does this NOT cover?

●     Appointments
●     Comprehensive lab result follow-up (while we will notify you of any critical abnormal lab results, to go over your labs and move your treatment plan forward, you will need an appointment)
●     Extensive research or genetic projects (these require multiple hours of investigations and will result in additional care fee hours being accrued or a single project fee can be arranged)
●     Referrals that require extensive interface such as with the Undiagnosed Disease Network or while outreaching multiple specialists and researchers
●     Unlimited out of visit consultation
●     Appointment cancellations with less than 48 hours’ notice

FEE DETAILS

Individual Membership Fee: $75 per month

Full Family Discounted Fee (2 or more members): $100 per month

Medicaid Holder Discounted Fee: $50 per month (to avail this discount a copy of Medicaid card is required)

Are there any discounts available?

There is a discount available for Medicaid patients as outlined above. We also extend discounts to patients who have 3 or more family members being seen at Lighthouse Complex Care.

Can I opt out of this fee? What if my case doesn’t require “out of visit” work this month?

No, you cannot opt out of this fee. Every patient at Lighthouse Complex Care requires more than an hour of case review before any appointment. We only accept patients that meet Complex Care Criteria in our clinic. It is only by charging this fee that we are ﬁscally able to oﬀer the standard of care we provide. We are fair when considering how much outside time your case has required on average.

What if I am no longer interested in being a Lighthouse Patient?

After agreeing to this fee when establishing care, you can terminate care at Lighthouse at any time. Your care fee will be charged for the month you give us termination notice, then discontinued the following month. For example, if you give us termination notice on March 1, you will still be charged on March 31. If you give us Notice on Feb 28th, your last care fee will be for the month of Feb.
