Haley's 23rd Annual SIDS Scramble is Monday, August 28, 2023 at Terradyne County Club (1400 Terradyne St, Andover, KS 67002). Please complete the following questions, if you are willing and able to volunteer.

Please wear country club attire (no denim or cutoffs). THE ENTIRE TOURNAMENT WILL BE OUTSIDE, SO PLEASE DRESS ACCORDINGLY.
