We/I the parent(s), guardian(s), of the above named athlete hereby give our/my approval for the athlete to participate in any all activities associated with the Bonivital Softball Fall Development Camp sessions and assume all risks and hazards incidental to the above named athletes participation, indemnifying, defending and holding harmless the Winnipeg Lightning Softball Organization, the Bonivital Softball Association, its Executive Members, Organizers, Sponsors, Coaches, Supervisors and Participants, from any claims, demands, actions, proceedings, lawsuits, fees, government imposed fines or penalties, liabilities, losses, damages, judgements, settlements, cost and expenses arising out of any actual or alleged injury or any person or damage to any property, claimed to be as a result, in whole or part, of the activities associated with the camp.
