First Teens - Halloween Event! Saturday, Oct. 28
Have a scary good time at our next First Teens event hosted at our Ben Hogan Learning Center at Squaw Creek! This is event will feature a long drive contest utilizing our Top Tracer Range technology, glow golf on the short course, a costume contest, a chicken finger dinner and some ghoulish treats! Come as you are or come in costume - we can't wait to see you there! 

*Please keep in mind the First Tee Code of Conduct and refrain from any costumes that could be considered offensive or inappropriate. Thank you!*

This event is open to all teens ages 13-18 who have been active with First Tee - Fort Worth within the last 12 months. 
First Tee staff will be on site at all times during this event.
No parent chaperones will be allowed unless they have completed the volunteer background check and training.

In the event that I (or my designated emergency contact) cannot be reached in an emergency, I agree to accept any and all determinations of need for medical assistance and/or administration of medical attention deemed necessary by First Tee - Fort Worth Chapter representatives. I hereby give permission to the medical personnel selected by First Tee - Fort Worth Chapter representatives to secure any and all medical, hospitalization, dental and/or surgical treatment. In the event that such medical attention is needed from a healthcare provider, all costs shall be the responsibility of the parent or guardian. 

Media Release
I hereby give First Tee - Fort Worth Chapter, Headquarters Office and participating agencies permission to use film, video tape and/or photographs of the above mentioned minor for lawful promotion or informational purposes. 

Hold Harmless
I, the parent/legal guardian of the above named youth, give approval for the participation in First Tee - Fort Worth activities. I assume responsibility for all risks of injury whatsoever and agree to hold harmless First Tee - Fort Worth Chapter and Headquarters Office from claim(s) of any nature arising from any activity, including transportation, connected with First Tee - Fort Worth facility or program. This Hold Harmless agreement includes but is not limited to, any claim due to injury proximately resulting from negligence of First Tee - Fort Worth Chapter or Headquarters Office, it's employees, agents, LPGA and PGA Professionals, participating agencies, and volunteers. I consent to First Tee - Fort Worth Chapter and Headquarters Office communication information regarding my child's participation via the Internet.
