This online form is to be completed by each prospective board member for Pine Springs Preparatory Academy and submitted along with the applicant's resume.

The purposes of this from are: 1) to supply pertinent information to the PSPA Nominating Committee; 2) to encourage potential board members to reflect individually as well as collectively on their common mission, purposes, and obligations as a board member; and 3) to identify any potential conflicts of interest you may have as a board member.

Upon completion of this form, the prospective board member should also email their resume to Shea Maliszewski, Board Chair, at:  smaliszewski@pinespringsprep.org

The Nominating Committee will review all candidates and will request an in-person interview with finalists.
