Travel Concierge Booking Form
For your convenience, the following services may be booked through our concierge.

a) VISA Application
b) Arrival Transfer & Departure Transfer
c) Travel Insurance
d) Pre-Departure Test
e) Air Ticketing

Kindly indicate the services you require below and we will proceed to make the necessary arrangements. Please take note that it is not compulsory to book the services through us and you may make your own arrangements if preferred.

The use of the booking services is subject to the following terms and conditions:

1. Upon submitting the booking request and where additional cost is applicable, Burnaby Solutions Pte Ltd (the merchant) will issue an electronic invoice in Singapore Dollars to confirm all arrangements for services booked. Online payment via credit card (American Express, Master card or Visa) will be required and subject to bank surcharge of 3.5%.

2. All credit card charges are made by the merchant's name - Burnaby Solution(s) Pte Ltd and in Singapore Dollars

3. For all services requested through this booking form, an administrative fee of S$19.26 net per participant (S$18.00 + 7% GST) shall apply. This administrative fee will be reflected in our final invoice to confirm all services.

4. Once additional services such as Travel Insurance and your Travel Visa are applied on your behalf by Burnaby Solutions, no cancellation or refund for unused services will be extended.

5. By submitting this form, you agree with the Terms and Conditions stated above and hereby confirm and authorize the appointed DMC Burnaby Solutions to provide these services.
