By checking the box below, I confirm I am the parent or legal guardian of the child whose name is given on this form, and I consent to my child receiving services from A2tor. I understand that A2tor cannot guarantee academic results or improvement for my student and that all A2tor tutors and leadership are high school volunteers and not professionally trained educators. I agree to have my child’s sessions recorded so they can be reviewed by A2tor Leadership, and I commit to communicating with my child’s tutor about important information regarding my child’s academics so the tutor can tailor lessons accordingly. I will communicate any concerns about my child’s participation in A2tor activities with A2tor leadership. Furthermore, I will respect the privacy of A2tor volunteers by not asking personal questions and not sharing the contact information of any A2tor volunteers with anyone outside of A2tor without the volunteer’s express consent. I further understand that if, at any time, my student’s tutor believes that my student no longer needs A2tor’s individual services, they will help my child transition into group sessions so as to share the resource of private tutors and thus benefit the community as a whole. *
