This form is to request a Hard Copy Learning Pack or Device if your children are going to be away;
* because they have COVID like symptoms (but no-one in the house is positive)
* for any other medical reason (like you usually would)
* because you as a family are being cautious about your children coming to school due to the number of COVID cases currently in the community

If you fill in this form you still need to record the absence on Hero as usual or leave a message on the school phone giving as much detail as possible.  You will need to do this every day that your child is absent from school unless you are able to give us an accurate timeframe of how long your child will be absent for in your initial message.  The more information you give us, the less chance we will need to contact you for more information.

PLEASE DO NOT FILL IN THIS FORM IF YOUR STUDENT HAS TESTED POSITIVE OR IS A HOUSEHOLD CONTACT DUE TO SOMEONE IN YOUR HOUSEHOLD TESTING POSITIVE.

If someone in your household does test positive at some stage while they are off school please let us know by filling in the appropriate form on our website.

When will the hard copy learning pack and/or device be ready for me to pick up?
* If the request is made between before 9am it will be ready for pick up that afternoon.  
* If the request is made between 9am and 3pm it may not be ready for collection until the next day.  
We are working as hard as we can to get these sorted as soon as we can for you.
