PMS Boat Dance 2024 Permission Slip
My child has my permission to attend the 8th grade Boat Dance on Friday, May 17, 2024 from 5:00 to 9:45 PM.

I understand that the Boat Dance will take place aboard the vessel operated by Hornblower Cruises. My child has permission to travel to and from the Empress Hornblower ship in the Berkeley Marina by chartered bus from Piedmont Middle School. I agree to deliver my child to Piedmont Middle School promptly at 5:00 pm for check-in and bus loading and understand that I must pick-up my child from Piedmont Middle School at exactly 9:30 pm.

I acknowledge that high heels are not allowed on the Empress Hornblower and students wearing high heels will not be permitted to board the bus.

I understand that Hornblower Cruises will not tolerate misconduct on the boat. I assume full responsibility for the behavior of my child at this event and understand that if my child’s behavior is inappropriate, disruptive, or indicates a health or safety concern while the boat is at sea, immediate action will be taken by a Parents’ Club Representative and/or chaperone to investigate and resolve the problem. I understand that in order to protect the health and safety of everyone on board the boat and to ensure that the participants are able to enjoy the Boat Dance, Parents’ Club Representatives and/or chaperones will take immediate measures to separate a disruptive child from other participants until that child is ready to rejoin the dance without further incident.

I hereby appoint any Parents’ Club Representative and/or chaperone to authorize medical treatment in case my child requires such treatment during the Boat Dance on Friday, May 17, 2024. I acknowledge, however, that no parent representative is a medical practitioner and that such representative and/or chaperone will summon professional assistance when apprised of a medical emergency. I hereby release and indemnify any and all Parents’ Club Representatives and/or chaperones from any and all damages with respect to any injury sustained by my child during the event. Any special medical needs, or any medication your child is/will be on during this event, must be reported to the school office prior to the event.

I understand that cell phones, purses, backpacks, bags and outside food, beverage, and water bottles are prohibited on board and will be confiscated before the bus departs and returned to the child at the end of the evening. I understand that students will be inspected for such items prior to boarding the bus.

I understand that students with two “U”s in Citizenship received on the May Progress Report may not be allowed to attend the Boat Dance. I also understand that this policy is a SCHOOL policy, monitored and administered by school officials only and any concerns or questions regarding this should be directed to Assistant Principal Angela Barrett.
