Futsal League Registration Form
League begins March 4th, regular season will conclude with a single elimination tournament
Games will be MONDAY NIGHTS, game times will be 6:00, 7:00, 8:00, and 9:00
Game times will be two 20-minute halves.
If signing up individually, put FREE AGENTS under Team Captain
Cost is $70 PER PLAYER.
Teams will provide their own jersey/shirts.
Need a minimum of 7 players, maximum of 10.

1. INDEMNIFICATION – To the extent allowed by law, the Participant, in consideration of its use of the facility, agrees to indemnify and hold Apopka Calvary Church of the Nazarene harmless from and against any liability of claims arising out of, or resulting from Renter’s use of the facility, and to pay Renter’s reasonable attorney fees, and those of Apopka Calvary Church of the Nazarene for defending such claims.

The undersigned is fully aware and voluntarily and expressly assumes the risk and other hazards inherent in the Activity and all other risks of loss, damage or injury to person or property that may be sustained by the participant or guests while participating in the Activity or on the premises of Apopka Calvary Church of the Nazarene.
