My parent/guardian hereby gives permission for me to be filmed, interviewed, recorded and/or photographed at any All- City Orchestra Rehearsal, Retreat, Performance or Workshop. The resulting photographs/video footage may be used in Philadelphia School District communications, publications and educational videos and those of All City’s Partner organizations (including, but not limited to, The Philadelphia Orchestra, Curtis Institute of Music, Project 440, Philadelphia Youth Orchestra, Temple University), and in School District publications and its cable station, PSTV Philadelphia 52, as well as by commercial radio and television stations and newspapers. In checking this box, my parent/guardian hereby releases any and all actions and claims which I, my parent/guardians, my family members, our heirs, executors or administrators may have against The School District of Philadelphia, All City, and/or All City’s Partner Organizations and any and each of its employees, representatives, agents, successors and assigns, arising for any reason whatsoever from the use, publication, distribution, or republication of the images and sounds gathered for the purposes mentioned above. My parent/guardian warrants that he/she is at least eighteen (18) years of age and has thoroughly read and understood this Waiver and Release paragraph..
