Certified Assisted Living Providers - $15/unit ($300 minimum) - A facility that provides assisted living services. Provide a list of these communities, and attach a copy of the certificate of occupancy or certification evidencing resident capacity. If a multi-facility provider, all facilities must join individually.

Affordable Assisted Living Providers - $300 - A facility certified by the State of Iowa as an affordable assisted lving provider and affordable to low and moderate income seniors.

Non-Certified Assisted Living Providers - 1-35 Units, $300, 36-74 Units, $500, 75+ units $750 - Licensed Residential Care Facilities, Independent Living for Seniors, nursing homes, hospitals, and any other senior housing/service/care alternatives, including developers. Flate fee based on resident capacity.
