Any information submitted will be seen by the Outreach Committee and the Conference Organizers and will be kept confidential.

Form responses will be reviewed nightly; we unfortunately don't have enough people on hand to immediately respond to non-emergency issues.

General conference feedback will be requested separately, please use this form to report in-conference issues only.
