CARES Regular Schedule
This form is intended for one time use by students who will follow the same Before Care and/or After Care routine each week. If you complete this form because your child's schedule will be consistent, then you do not need to complete the Weekly Enrollment Form.

This form will be shared with each child's teacher, and it will serve as dismissal authorization, if applicable.

*Changes to this form should be the exception not the norm.  Email changes to your child's teacher as well as the CARES director (cares@holyfamilyaston.org).

IMPORTANT: Late day change of dismissal notifications must be directed to the school office and instructions left with a live person.  Never leave late day changes on voicemail or email-- they might not be seen in time for dismissal.
