COVID-19 Daily Health Check Declaration - For Players/Opponents/Officials/Supporters/Bar Visitors at any match played at Harrow St Marys Cricket Club.

All players and those accompanying them are required to declare their health status daily before entering Harrow St Marys Cricket Club and its grounds.

This form must be completed ON THE DAY you attend the club, not before. You will not be admitted to the Club unless you have completed this Declaration on the day you attend the Club.

If you are unable to complete and sign the Declaration because not all of the statements apply to you then you will not be able to enter the Club.

Any complete declarations provided in advance will not be accepted. Please fill this in ON THE DAY that you will be attending the Club.

The Club (and its representatives) reserve the right to refuse entry to the Club or ask anyone to leave the Club (including its grounds) if this form is not completed for each day of entry at the Club and/or the Club Representative consider that the person entering the Club grounds does not meet the declaration points below or is displaying any symptoms associated with Covid-19.

Data captured by Harrow St Marys Cricket Club will be deleted after a set number of days as per government and ECB guidelines.
