NDPP Curriculum Request Form

The Chronic Disease Prevention Program (CDPP) at the Wyoming Department of Health has received a large number of requests for free NDPP curriculum. To help prioritize requests, the CDPP is requiring information from program sites about their NDPP timeline and need for assistance. Please complete this form in order to be eligible for free NDPP curriculum. The form will take less than 10 minutes to complete.

Please note that free curriculum materials are available only to sites that are pursuing CDC-recognition or that have already achieved CDC-recognition of their DPP. Distribution of materials will be determined based on need and program start date.
