Service Dog-Assisted Interventions in Parkinson's Disease / Lewy Body Dementia

Thank you for taking the time to complete this survey. It will take approximately 15-30 minutes.

The purpose of this questionnaire is to establish the level of interest among Parkinson's disease (PD) and Lewy body dementia (LBD) patients and their caregivers in pursuing therapeutic support with service dogs. For those who already have a service dog, we wish to determine their effect on patient and caregiver wellbeing.

*Note: A service dog refers to a dog which has been especially trained to perform a function or job for someone with a physical, intellectual, and/or emotional disability. This differs from an emotional support dog.
