This form must be completed by both the PARENT/GUARDIAN and the STUDENT. The committee will randomly select forms for signature authorization. Any permission slips that we are not able to verify completion by both the parent/guardian and the student will result in the student not being aloud to attend the Grad Night event.

Grad Night 2020 Edison High School Statement of intent:                                                         Congratulations to your student on their achievements for the school year! On June 6th in order to ensure an incredibly fun and safe atmosphere, we have the following list of prohibited items. We ask your help as parents and guardians to ensure none of these items are brought to Grad night. If you have any questions regarding grad night, please contact the grad night chairperson at your school. Please complete and return this form to your grad night chairperson at the time of ticket purchase, thank you. In partnership with the Anaheim Police Department, we will also be conducting screenings for banned substances. Screenings will include bag searches and the use of drug-sniffing canines. If bag searches and/or the drug-sniffing canines indicate the presence of an illegal substance, we will ask the subject student or chaperone for permission to conduct a pat-down search. If any banned substances are found on the bus or in the possession of a student or chaperone, the matter will be turned over to the Anaheim Police Department for further action. Schools/groups that violate this policy are subject to be banned from future Grad Nite events. Attending Grad Nite is not a right, it’s a privilege. My student (listed below) will be attending grad night 2020. I understand that certain rules and regulations will be in effect during grad night, as listed here and below, and I agreed to review and advise my student of such rules and regulations prior to grad night. I also acknowledge and agree that in the event my student violates any such rules or regulations, my student will be required to exit Grad Night via parental pick up without any possibility of refund. I agreed to be the responsible person for picking up my student if any violations occur and my student is required to exit Grad Night via parental pick up without any possibility of refund. *

Grad Night Rules and Regulations: The following items are prohibited and all students will be subject to search prior to entering Grad Night - 1. Alcohol and drugs (controlled substances). 2. Tobacco products (cigarettes, cigar, vapes, chew, etc.) and all related items (lighters, matches, etc.). 3. Any non-prescription medication including aspirin and vitamins. 4. Contact solution, eye drops, Hand sanitizer larger than 4 ounces or 120 mL. 5. Any type of weapon, aerosol can, markers, sharpies, pens or laser pointers. 6. All fluids of any kind, including water, drinks, perfume, cologne, lotions. 7. All glass containers of any kind such as perfume, cologne. 8. Any shoes with skate wheels on them or high heeled shoes. 9. Any headwear or costumes. 10. No spiked jewelry or belts. 11. All bags, purses, and backpacks must be smaller than 8 1/2 by 11”. 12. Any and all prescribed medications will be surrendered to your Grad night chairperson for safekeeping and picked up at the conclusion of the event. 13. We allow only ball caps and religious headwear as defined by law. Ball caps must be worn front or back facing. 14. No hats, clothing, jewelry or visible tattoos with offensive language or imagery. 15. Sunglasses may not be worn in the premises. 16. We do not allow clothing which exposes underwear or excessive portions of the skin, which may be viewed as inappropriate in a family environment *

To ensure a safe and enjoyable experience for all Grad Nite attendees and other park guests, please respect the following rules. Excessive celebratory behaviors that can be interpreted by other guests as intimidating and/or disruptive are prohibited at the Disneyland® Resort and during the Grad Nite event: • Groups walking and/or running in locked arms (this can appear threatening to other guests and is also a safety concern in impacted areas). • Group chanting of any kind including yelling at high volumes in attraction queues. The following behaviors and activities are not tolerated at the Disneyland® Resort: • Inappropriate touching or physical aggression of any kind.• Failure to comply with a request of a Disney Cast Member (our Cast Members are trained to direct our guests for optimal safety and to comply with our standards of conduct). • Inappropriate language (including profanity) and disruptive or offensive behavior.• Disrespectful demeanor toward other park guests, Disney Cast Members or other law enforcement officials.• For your safety and that of others, please refrain from running.• Theft or vandalism. • Line jumping/cutting in attraction queues. • Failure to comply with attraction safety rules• The distribution of printed or recorded materials of any kind• Photography, videotaping or recording of any kind of commercial purposes. • Feeding any animals in the park, including birds. *

The following items are prohibited for all Grad Nite attendees (students & chaperones): • Alcoholic beverages or any illegal substances. • Medical marijuana, even if it has been legally prescribed to the Student or Chaperone. • Glass containers of any size or type including perfume/cologne bottles and reusable water bottles. • All outside food and liquids including factory-sealed water bottles. • Weapons of any kind, or objects that appear to be weapons. • Laser pointers or other items deemed potentially harmful or disruptive including Bluetooth speakers. • Smoking, vaping and electronic cigarettes. • Selfie sticks (hand-held extension poles for cameras and mobile devices). • Suitcases, coolers or backpacks with or without wheels that are larger than 24” long x 15” wide x 18” high. • Musical instruments. • Large tripods and professional photography equipment and lighting. • Recreational devices such as drones, remote control toys, skateboards, scooters, inline skates, shoes with built-in wheels. *
