Clients Rights and Informed Consent:

Career counseling is a process where you will enter into a relationship with a trained professional to gain greater insight into your current situation and to develop more effective ways of setting and reaching your goals. Career counseling is an interactive process.  Your counselor will not give advice or make decisions for you, rather, they will help you understand yourself more fully, reach your own conclusions, and make your own decisions.

In career counseling, facts, feelings, and emotions will be discussed. Your counselor will treat what you tell him/her with great care and professional ethics prevent the sharing of this information with anyone without your permission.  In the event of an emergency, defined as clear and imminent danger to yourself or others, the law may require your counselor to share your information with others.  If you have any questions about confidentiality, please talk with your counselor.  You have a right to terminate counseling at any time, refuse treatment and may seek a second opinion of another counselor or file a complaint without retaliation.

Our facility also provides training for masters and doctoral level counselors.  As part of this training, some sessions may be video recorded.  We will not record your session without first obtaining your written permission.  All recordings will be erased after they are reviewed by supervisors who are Licensed Professional Counselors. In order to provide the best possible service, your counselor may consult with a qualified supervisor or another professional staff member about your case.

I have read the information above.  I understand that I will have an opportunity to discuss any questions regarding services with my counselor.  I agree to all above procedures and give consent to services at the Career Center.
