By typing your name below you are acknowledging your agreement with the following liability statement: "As the parent/legal guardian of the above-named minor(s), I give my permission for him/her to participate in any and all activities, events, and programs at the Rock City Church Vacation Bible School. In the event of a medical emergency, I authorize a representative of Rock City Church to transport my child to the nearest medical facility for treatment or call 911. By signing below, the participant (or parent/guardian, if participant is a minor) acknowledges and accepts the risk of illness or physical injury associated with participation in the activities described above. I accept personal financial responsibility for any illness or bodily or personal injury sustained during the activity. Further, the participant (or parent/ guardian) promises to hold harmless the Rock City Church and its representatives for any illness or injury related to the activity. I understand that I am fully responsible for all legal and court costs should I pursue action against Rock City Church and/or its representatives. I understand that registration fees collected are not refundable for any reason. *
